A. All persons allowed benefits from the fund, whether adult or minor, must be an enrolled member of the Osage Nation.

B. Any member, whether adult or minor, parent or legal guardian, must submit a completed application/enrollment form annually during the enrollment period between October 1st and December 15th to access benefits for the following calendar year. Applications may be accepted past the enrollment deadline upon applicant experiencing a life event of the birth of a child, the legal adoption of a child or being named a guardian of another person by a court of law.

C. Over sixty-five (65) members enrolled in Medigap coverage will automatically be extended coverage from one year to the next unless the provider is notified of death or desire to discontinue the coverage in the method designated by the provider.

D. Members who have been convicted in the Osage Courts of fraudulent use of benefits from the Health Benefit Fund are ineligible to access benefits for the period described in the judgment.

E. Each enrolled member shall have an individual account established. The account shall reflect the current year’s balance as well as rollover balances. ONCA 08-46, eff.
