A. All workers shall disclose any preexisting physical or mental disorder and/or condition that could potentially affect or impair the worker’s ability to perform in a reasonable and safe manner the activities involved in the position in which they work. Disclosure shall be made in writing before commencing job duties or before a change in job duties.

B. Any claim resulting from an employment related aggravation of a preexisting condition which was not disclosed as required under this Act shall be declined by the Administrator under this Act if the claimant had knowledge of the preexisting condition and failed to disclose such condition pursuant to subsection (A) of this section.
