A. When a worker’s attending physician believes that palliative care is appropriate to enable the worker to continue current employment or a current vocational training program, the attending physician must submit to the Administrator a request for palliative care. The request must be submitted within sixty (60) days of initial claim closure. The request shall:

1. Describe any objective findings;

2. Describe the medical condition for which palliative care is requested;

3. Detail a treatment plan which includes the names of the provider who will render care, specific treatment modalities, and frequency and duration of the care not to exceed sixty (60) days;

4. Explain how the requested care is related to the compensable condition;

5. Describe how the requested care will enable the worker to continue current employment or a vocational training program.

B. When received and date stamped, the request will be reviewed by the Administrator. After such review, the Administrator decision on the request will be finalized and mailed to the worker within thirty (30) days of the date of such request. ONCA 12-76, eff. Oct. 1, 2012.
