A. To receive a photo ID issued by the Osage Nation, the applicant must present to the Osage Nation Tax Commission the following valid documents:

1. Birth certificate;

2. Social Security card;

3. Certified Degree of Indian Blood card or Osage Nation Membership card.

B. There are no residency requirements for receiving a photo ID.

C. A photo ID issued by the Osage Nation will contain the following on its face:

1. Seal of the Osage Nation;

2. Full legal name;

3. Address;

4. Date of birth;

5. Height;

6. Weight;

7. Eye color;

8. Date of expiration;

9. Photo ID number (to be matched with Osage Nation membership number).

D. Photo IDs will be valid for a period of five years from the date of issuance.

E. Issuance fees are due in full at the time of application in a form of payment deemed acceptable by Osage Nation Tax Commission policy. ONCA 12-58, eff. Apr. 19, 2012.
